Bilateral Symmetrical Periarticular Trauma in Epileptics

Authors: RAYMOND BELLAMY MD, DAVID B. STEVENS MD

Abstract

AbstractTwo cases of bilateral symmetrical periarticular trauma in epileptics are presented, one with bilateral posterior dislocations of the shoulder and one with bilateral subcapital femoral fractures. The implications for future diagnosis and management of this syndrome are discussed. Although the history may be confusing, bilateral symmetrical periarticular trauma usually is due to convulsive seizures. Because of the symmetrical physical and roentgenographic findings and the frequently confusing history, correct diagnosis may be difficult. Although modifications in the usual treatment approach may be necessary because of delay in diagnosis and the possibility of further seizures, early diagnosis of the orthopedic problem and control of seizures are the most important factors in the outcome.
