Sylvia Coleman, President and CEO of Sylvia Coleman & Associates, has over 30 years of experience in community and public health. Her work focuses on integrating health and human services to improve person-centered care and population health outcomes. Her roles include serving as a Fellow of the American Heart Association, nurse researcher, health policy advisor, strategy consultant (Constellation Quality Health) and CHW ally (HEAL CHW Learning Collaborative). Sylvia serves on state/national committees, including the Partnership to Align Social Care (Billing and Coding), and consults for NC DHHS. She holds a BSN, MPH, Gerontology Certificate, and is a Legal Nurse Consultant.
