Overview

This activity equips healthcare providers and interdisciplinary teams with the knowledge, skills, and tools to implement whole person health approaches in clinical and community settings. The course focuses on integrating clinical care with social care, addressing barriers to care, and applying evidence-based strategies to improve hypertension and cardiovascular outcomes across diverse populations.

Learning Objectives

Upon completion of this activity, learners will be able to:

  • Define whole person health and describe its relevance to hypertension and cardiovascular disease outcomes.
  • Examine the impact of social drivers of health and structural factors on cardiovascular disparities.
  • Apply culturally responsive, patient-centered communication strategies to improve patient engagement and adherence.
  • Implement standardized workflows to screen for and address social needs within clinical practice.
  • Integrate community-based partnerships and team-based care models to strengthen care coordination.

Credits Available

Southern Medical Association designates this enduring material activity for a maximum of 6.0 AMA PRA Category 1 Credits. Physicians should claim only the credit commensurate with the extent of their participation in this activity.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Southern Medical Association and Constellation Quality Health. Faculty

Maxine Arena, MPH, MBA
Maxine is an experienced public health leader with over 13 years of work focused on advancing health equity and improving outcomes for underserved populations. Her expertise spans community-based interventions, chronic disease prevention, program management, health promotion, and system-level health policy reform. Throughout her career, she has successfully navigated complex healthcare systems to design and implement strategies that address the root causes of health disparities.

Benita Arokiaraj, MPH
Benita brings four years of experience in health equity programming, strategic communications, and providing technical assistance. She has engaged with cross-functional teams and community partners across academic, federal, and nonprofit sectors to deliver impactful messaging and process improvement strategies, driven by a passion for advancing community well-being through innovative and effective solutions.
