An 80-year-old female with a past medical history of a nonfunctioning 4 cm pituitary macroadenoma and a recent DVT on anticoagulation with apixaban presented to the ER with acute onset nausea, vomiting, and a severe headache that started after a fall in which she hit her head.

Tacrolimus-Induced Pericardial Effusion
Tacrolimus, calcineurin inhibitor, is an immunosuppressive therapy for transplant rejection prophylaxis. It has effects such as nephrotoxicity, neurotoxicity, infection, ascites and pleural effusions. However, pericardial effusion is an effect rarely seen. We present a case of a patient with recurrent pericardial effusion associated with chronic treatment with tacrolimus.

Autoimmune myopathies are rare inflammatory conditions that share a common feature of immune-mediated muscle injury. The estimated prevalence of polymyositis is 5 to 22 per 100,000 persons. Infection, malignancy, and cardiovascular accidents are the leading causes of death in patients with polymyositis.

There’s more to ICU than COVID-19: Simulation based training for core trainees
The COVID-19 pandemic saw many anaesthetic trainees, regardless of their level of training, being redeployed to assist in Intensive Care Units (ICU). This has skewed the first ICU exposure for the Core Trainees (CT1/CT2) who completed their ICU module requirements with limited exposure to the pre-pandemic regular mix of ICU cases.
