Commentary on "Project BOOST Implementation: Lessons Learned"

Abstract

In this issue of the Southern Medical Journal, Williams et al present their experience with the implementation of a patient discharge toolkit (Project BOOST [Better Outcomes by Optimizing Safe Transitions]) as part of a quality improvement (QI) initiative aimed at reducing hospital readmission.1 The authors qualitatively assessed implementation of the toolkit elements across several hospitals during three different rollouts of the project and describe barriers to implementation of the QI project as well as strategies to overcome the barriers. Specifically, the authors focus on the role of the external physician mentor and posit that implementation of key components of Project BOOST improved with increases in the amount of formal contact between the external mentor and the internal team.
