The Tympanomastoid Fissure: A Reliable Approach to the Facial Nerve in Parotid Surgery

Abstract

Abstract: A reliable method of identifying the facial nerve is of paramount importance in operations for benign tumor of the parotid gland. A review of 168 cases strongly supports the tympanomastoid fissure as the safest, most constant landmark in experienced as well as inexperienced hands. A brief comparison of the TM fissure versus other landmarks is made. Several factors that may contribute to postoperative paralysis are discussed. Certain principles are recommended to protect the integrity of the facial nerve in benign parotid surgery.
